How it is calculated
Berlin definition requires all four: onset within 1 week, bilateral opacities not fully explained by effusion or collapse, respiratory failure not fully explained by cardiac failure or fluid overload, and P/F 300 or below on PEEP of at least 5. The p/f ratio alone is not ARDS - the other three criteria do the diagnostic work.
