How it is calculated

CHA₂DS₂-VASc for stroke risk in atrial fibrillation. Female sex modifies risk rather than creating it: the advice is based on the other points, so anticoagulation is considered at 1 point for men and 2 for women, and recommended at 2 for men and 3 for women (ACC/AHA 2023). The 2024 ESC guideline drops the sex point altogether (CHA₂DS₂-VA). The percentages shown are the adjusted annual stroke rates from the Lip 2010 validation cohort, which is the table most references reproduce. Above a score of 6 no figure is quoted: the published cohorts disagree by several percentage points at the top of the range and the highest scores rest on very few patients, so a single number to one decimal place would be false precision. The decision to anticoagulate does not turn on that digit.
