How it is calculated

HOMA-IR = fasting glucose (mmol/L) x fasting insulin (mIU/L) / 22.5. It is a research and population tool rather than a diagnostic test - cut-offs vary between populations and assays, and it is not valid in anyone on insulin or with significant beta-cell failure, where the fasting insulin no longer reflects resistance.
