How it is calculated
Modified Duke criteria. Definite requires 2 major, or 1 major and 3 minor, or 5 minor. Major: typical organism in two separate cultures (or persistently positive cultures, or a single positive for Coxiella burnetii), and evidence of endocardial involvement on echo or a new regurgitant murmur. Minor: predisposing heart condition or injecting drug use, fever above 38 degrees C, vascular phenomena, immunological phenomena, and microbiological evidence not meeting a major criterion. Pathological criteria - organisms or lesions demonstrated in the vegetation itself - are definite regardless of this tally.
