How it is calculated
Modified Duke criteria (ESC 2023). Definite requires 2 major, or 1 major and 3 minor, or 5 minor. Major: typical organism in two separate cultures (or persistently positive cultures, or a single positive for Coxiella burnetii), and evidence of endocardial involvement on echo, cardiac CT or PET/CT, or a new regurgitant murmur. Minor: predisposing heart condition or injecting drug use, fever above 38 degrees C, vascular phenomena, immunological phenomena, and microbiological evidence not meeting a major criterion. Pathological criteria - organisms or active endocarditis in a vegetation, an embolised vegetation or an intracardiac abscess - are definite regardless of this tally. The 2023 Duke-ISCVID criteria are the US/international alternative.
