How it is calculated

Osmotic demyelination syndrome follows over-rapid correction and is irreversible. Chronic hyponatraemia (over 48 hours, or unknown duration) is the high-risk group; if the duration is unknown, treat it as chronic. With severe symptoms at any duration, give 150 mL of 3% saline over 20 minutes, repeated, aiming for a 5 mmol/L rise, then slow correction (European guideline 2014).
