How it is calculated

qSOFA: RR >= 22, altered mentation, systolic BP <= 100. It is a prognostic prompt, not a diagnostic test; qSOFA alone is not recommended for screening (Surviving Sepsis 2026 prefers SIRS, NEWS2 or MEWS), and a score under 2 does not exclude sepsis.
