How it is calculated

Sgarbossa criteria identify acute MI in the presence of LBBB or a paced rhythm, where ordinary ST criteria fail. It is specific but not sensitive - a score under 3 does not exclude infarction, which is the commonest misuse. The modified criteria replace the 5mm discordant threshold with a ST/S ratio of -0.25 and perform better.
