About

Gentamicin concentration immediately before the next dose. With once-daily dosing a LOW trough is the goal: it shows the kidneys had time to clear the drug.

When it is low

A low trough is the goal with once-daily dosing - it allows the concentration-dependent killing to work while letting tissue levels fall

When it is high

Accumulation: irreversible vestibular toxicity and nephrotoxicity. The trough matters more than the peak for toxicity
