Average glycaemia over about three months. Falsely LOW where red cells are young or short-lived, falsely HIGH where they are old or turnover is slow.
When it is low
falsely low where red cells are young or short-lived: haemolysis, recent blood loss or transfusion, pregnancy, erythropoietin therapy, haemoglobinopathies
When it is high
Diabetes: 5.7-6.4% prediabetes, 6.5% or above diabetes. Reflects a 2-3 month average. falsely high where red cells are old or turnover is slow: Iron, B12 and folate deficiency, chronic kidney disease, splenectomy, alcohol - correct the deficiency before escalating diabetes treatment on the number
