A morning cortisol, timed to the natural peak. A low value raises the question of adrenal insufficiency; a high one, of Cushing syndrome.

When it is low

Adrenal insufficiency - confirm with a Short Synacthen test. Consider suppression by exogenous steroids

When it is high

Cushing syndrome, stress, acute illness. A single random value is rarely diagnostic
