Serum valproate, checked for adherence and toxicity. Carbapenem antibiotics drop it sharply.

When it is low

Subtherapeutic for seizure control

When it is high

Tremor, sedation, hyperammonaemia. Carbapenems drop levels by ~60% within 24 hours and raising the dose does not compensate
