Vancomycin concentration immediately before the next dose, used to keep the drug effective without damaging the kidneys.

When it is low

Subtherapeutic: risk of treatment failure and resistance

When it is high

Nephrotoxicity risk, compounded by concurrent aminoglycosides, NSAIDs or contrast. Troughs of 15-20 usually overshoot the AUC target of 400-600; US guidance prefers AUC-guided dosing
