Conscious level scored across eye, verbal and motor responses. The scale runs from 3 to 15. Always report the three components, since a total of 9 can mean very different things.

When it is low

≤8: Severe impairment, cannot protect airway. 9-12: Moderate. 13-15: Mild

When it is high

15: Fully alert, the top of the scale

How it is measured

E (1-4) + V (1-5) + M (1-6). Document as E_V_M_. GCS ≤8 typically requires intubation.
