When it is low

Mild or no pain - reassess periodically

When it is high

Moderate (4-6) or severe (7-10): Requires intervention. Consider analgesic ladder

How it is measured

Use validated scales: nrs (0-10), Wong-Baker faces, flacc (pediatric), bps (sedated).
